Documentation Guidelines

Students must submit comprehensive documentation of their disability, including a diagnostic statement from a qualified professional in the appropriate discipline. Documentation should address how the disability currently impacts the student’s access to the college physically or academically.

Individual Education Plans (IEP’s), Section 504 plans, and Summary of Performances (SOP’s) are useful in helping the student request accommodations at the college level. Students should refer to the specific guidelines listed for acceptable documentation.

Documentation to Provide

Documentation should be on letterhead and provided by a qualified, licensed professional whose field of specialization directly pertains to the student’s disability and who is unrelated to the student. IEP or 504 plans are accepted if the information provided follows the below guidelines.
